HealthConnections

Since 2009, we have been committed to offering integrated healthcare for members through our HealthConnections program. This program started as a pilot effort aimed at integrating physical and behavioral health services for adult Medicaid beneficiaries with serious mental illness and co-occurring physical health conditions.

Our efforts have evolved to include a well-established Wellness Recovery Team in Montgomery County and provider-level Nurse Navigators in Bucks county. In April 2017, we further expanded our program with the creation of a new Wellness Recovery Team to support integrated health efforts in Lehigh and Northampton counties.

While there is more than one model of health integration across the counties, all programs and initiatives have a common focus of providing individualized health integration and improving a member’s overall wellness.
