We are committed to promoting integrated care in our communities and building bridges with medical practitioners in an effort to improve care coordination and Member health outcomes. This page is designed to give medical practitioners and prescribers the tools needed to assess behavioral health and provide referrals to resources.
Primary Care Physician? – Talk with a behavioral health expert
PCP’s can request a consultation with a Magellan medical director by calling Magellan’s Provider Line at one of the County phone numbers below, and identifying yourself as a treating medical provider. Remember to complete an AUD form prior to a consultation, and the AUD form is available here.
Remember, you don’t have to do this alone. By striving to improve care coordination through our PCP toolkit, we will continue to promote quality care for Magellan Members.

Magellan supports members of our community who are pregnant or postpartum. We have maternal health Care Managers who support making connections to resources, whether it be behavioral health, physical health, or other resource needs. These resources are available at no cost. Stop Smoking
Quitting smoking isn’t easy. But if you smoke, quitting is the best thing you can do for your health. Smoking increases your risk for cancer, diabetes, heart failure, and other serious medical problems. Still, if you’ve been smoking for a long time, it can be very hard to quit. You might worry about withdrawal symptoms or that your craving for a cigarette might be too strong to ignore. But with some support, you can do it. Our resources can give you the help you need to finally quit smoking and start leading a healthier life today.
Community HealthChoices
Community HealthChoices (CHC) is Pennsylvania’s Medical Assistance managed care program that includes physical health benefits and long-term services and supports (LTSS). CHC serves individuals who also have Medicare coverage, and disabled adults age 21 and over.
Wellness Recovery Teams and Nurse Navigators
Since 2009, we have been committed to offering integrated healthcare for Members through our HealthConnections Program. This program started as a pilot effort aimed at integrating physical and behavioral health services for adult Medicaid beneficiaries with serious mental illness and co-occurring physical health conditions.
